Good Faith Estimate
Your Right to a Good Faith Estimate
Anchored Mind Counseling, LLC
Understanding Your Right to Know the Cost of Therapy
Under the federal No Surprises Act, individuals who do not have health insurance or who choose not to use their insurance to pay for healthcare services have the right to receive a Good Faith Estimate explaining the expected cost of their care.
A Good Faith Estimate provides an overview of the anticipated charges for healthcare services before those services are provided.
At Anchored Mind Counseling, LLC, we believe you should have clear information about the financial side of therapy so you can make informed decisions about your care.
Who Is Eligible for a Good Faith Estimate?
You have the right to receive a Good Faith Estimate if you:
Do not have health insurance.
Have health insurance but choose not to use it to pay for therapy.
Are paying directly for services rather than submitting claims through your insurance.
You may request a Good Faith Estimate before scheduling an appointment or at any time during treatment.
When Will I Receive My Estimate?
Federal requirements establish the following timelines:
Appointments scheduled 3–9 business days in advance: You must receive a written Good Faith Estimate within one business day after scheduling.
Appointments scheduled 10 or more business days in advance: You must receive a written Good Faith Estimate within three business days after scheduling.
Estimates requested without scheduling an appointment: You must receive a written Good Faith Estimate within three business days of your request.
You may request an estimate in paper or electronic form.
What Will My Good Faith Estimate Include?
Your individualized Good Faith Estimate will outline the reasonably expected costs of your therapy services, including the anticipated services, frequency of sessions, and estimated charges for the applicable period of care.
Current standard session fees at Anchored Mind Counseling are:
Individual Therapy — 60 minutes: $150
Couples Therapy — 75 minutes: $165
The total estimated cost will depend on the type of service, anticipated frequency of appointments, and expected duration of treatment.
Your Good Faith Estimate is not a bill or a contract. It does not require you to receive services, and actual charges may differ if your treatment needs or services change.
Additional services that were not reasonably anticipated when the estimate was prepared may not be included.
What If My Bill Is Higher Than My Estimate?
If you receive a bill that is at least $400 more than the amount listed on your Good Faith Estimate for a particular provider or facility, you may have the right to dispute the charges through the federal patient-provider dispute resolution process.
You may also contact Anchored Mind Counseling directly to discuss any questions or concerns about your bill.
To initiate the federal dispute process, you generally must submit your dispute within 120 calendar days of receiving your initial bill.
An independent third party will review the information and determine the appropriate payment amount.
Initiating a dispute will not negatively affect the quality of care you receive.
Keep a Copy of Your Estimate
We encourage you to save your Good Faith Estimate and compare it with any bills you receive.
You may request another copy of your estimate if needed.
